Three pathways of development of diseases are described in Charaka Samhitaa:
1. Baahya – Rakta – other Dhaatu and Tvak ( skin ) constitute the Baahya Roga Maarga ( external pathway for disease ). From it arises diseases such as moles, discoloured patches, Gan`d`amaalaa, ( glandular ) ulcers of face, malignant tumours, Haemorrhoids etc.
Abhyantara – Aamaas`haya, Pakvaas`haya ( large intestine ) also known as Antah Kosht`ha ( internal viscera ) and Mahaa Srotas constitute the internal pathways. Vomiting, diarrhoea, cough, dyspnoea, Ascites, fever, dropsy, Abdominal tumour etc. arise from this pathway.
3. Madhyama – The head, heart, urinary bladder, and such other vital points, joints of bones, the veins, Snaayu , big tendons constitute the Madhyama Maarga ( middle pathway ). Hemiplegia, facial paralysis, diseases of head and other organs are the diseases arising from this pathway.
The prognosis also differs depending upon the pathway chosen by the vitiated Dosha to traverse.
